I have added as much as 4 tablespoons per 5 gallons with success in getting rid of the ich and not harming any fish. I have also had success with 3 tablespoons per 5 gallons. This with the temp at 84 degrees. Keep in mind, this was for fish in general, not Pbass.
 
One more question the boxs does not say how often to use the salt. Do I add that much daily? And once the fish get better do I still use the salt?
 
Do waterchanges and only add more salt to the new water you're adding to the tank. Daily addition of salt for entire tank volume would be bad! Just remember that salt only goes away with water changes. Good luck!
 
Salt is extra... helps with breathing.. but not necessary...

warm water at 86 degrees MAX!!!! is key, and good water change with ich meds.... i can usually cure it in 7-9 days...

why is 86 max?
 
Raising the temperature only speeds up the ich life cycle, 86 should be fine. the salt is to keep the fish from getting too stressed from the disease. If your tank is not already at 86 I would suggest slowly raising it in 2 degree increments. Buy a good ich medication from your lfs, and add it according to the directions. Most ich medications will require you to remove the carbon from your filter. By raising the temp you speed up the ich life cycle, and are able to kill it with medication once it's in the tomite stage.
