Seems like he may be having an issue with his slimecoat. Perhaps even a fungal issue.
Do the white areas on his disc look fuzzy?

looks abit better mate. keep up the feeding as much as he will eat as often as possible.
im sure youll pull him threw if you stick with it.
and yes im going to say it again, when he dose get bigger please upgrade your tank and he will do much better.
