Whenever I hear about a fish getting out of the tank even with lids on it makes me think about my own tank. How do these guys get out, the only way is to jump and it does not seem likely they could be accurate to jump through a specific opening in the tank especially with the lid, filter, heater, etc. I know my ID is a great jumper, I've only seen him jump when I had him in a low plastic tub prior to transport. In the main tank I'm confident he will never jump out, same goes for my leporinus, another great jumper. How deep must a fish be in order to get a running start to a good jump ? I would think just by swimming at the top alone they wouldn't be able to jump or could they ? I recently got a Bichir, I hear these guys get out of tanks with lids on them too, based on initial observation I just don't see it. Do fish plot and plan their escape, knowing which areas of the lid have open areas ?
I've seen a 12" fish hit about 16" out of a bucket with only about 3-4" of water in it. The bucket wasn't wide enough for the fish to stretch out to full length.
I've also known of one 12" ID that managed to gain enough speed in a 36" tank to go through the glass at the opposite end. You might want to rethink your assumptions on how much speed they can get in a short space.

Did he have lightening shoot out his backside too ? I definitely heed the warnings, just seems like a fish would need to have a good reason to jump out of a tank like that, stress, etc. Makes me nervious now everytime I take the lid reading some of these stories. So far I've had no fear of my ID or my new Aro or Bichir jumping, they've had many opportunities with the lids off.
Will consider myself lucky...
I would be curious how big was your Bichir and how big was the opening in your top ? He looks pretty large so the opening would have had to be rather big no ?
