This is a really old thread. But...

No, but close. I had a large male managuense hit the side of the tank so hard he paralyzed himself. I did the best I could for him, but after a month of no recovery and no eating, I had to put him down. A large Oscar that was in with him barely left his side and would try to nudge him off the bottom.

The answer is yes, that has happened. Most large fish are capable of doing this.
 
I too have lost an oscar due to jump out. I always cover my tanks with glass tops but this guy got out about a 1" gap between the HOB filter and the plastic cover trim of the glass top. He was around 5" so I thought it was safe. Now I make sure all my cut outs are very secure with almost no extra space so this will not happen again.
 
I have had 1 oscar jump out and thank God I was home. I just heard him flopping on the floor and put him back in the tank. But I did lose a male Jag. Found him dead the next day:(

No, but knocked himself out hard enough after being spooked at night by a Vieja to float unconciously for several minutes, and i though he was a goner...
But he regained conciousness and darted around senseless for some more minutes, but healed up allright after some weeks.
The bully was rehomed the next day...

the dark stripes around his eyes were the bruises he had from hitting the lid, plus skin missing...
