When I was a kid I had a 7" oscar in a 15 gallon tank. I just cleaned every 2 week (100% becuase at that time I did not know the 100% rule) The water was clean because I used a filter that claimed to do 300 gallons per hour. To make it short JUST USE A BETTER FILTER!!!!
 
Some times you get them from crappy shops that way, my oldest is prone to it for life due to the quality of its life prior to coming home with me!!
Seriously mate if its that bad: METRONIDAZOLE i've my hit my O with it once and its never come back!!!!
 
90% WC? is that done in separate sessions or just one big WC?
 
Who says that a disease that is caused by a parasite is the cause of malnutrition?
I have been told that Hole in the Head is caused by a combination of flagellated protozoans and bacteria.
I think the best thing to do if you buy a fish that is prone to this disease is to get a uv sterilizer, change that god awful smelling filter cartrige and replace it with some cheap cut to fit filter pads and not continue to stress him out with your 90% water changes. I have never tried using a uv sterilizer but it's worth a shot if you continue to buy a turd with wings as the last poster said.
 
90% water change is too much. Charcoal does not last indefinitely, it gets saturated and then leeches toxins back into the water. Biological filtration is best IMO, charcoal is chemical filtration. Hikari pellets, smelt, prawns is all mine have ever had, and they have never suffered with HITH. I have a pair, 12 inches each, and 2 singles 10 and 11 inches. Been keeping oscars for the past 30 years, good filtration 20% water change per week is all I have ever done.
 
In the Discus world on Simplydiscus we do not use carbon for the specific reason we believe that it contributes to HITH. We use it to take out things in the water such as medications and so forth.

It is believed that carbon does more harm than good as it takes out important nutrition in the water.

HITH is a stress induced disease caused by an intestinal flagellate protozoan, usually Hexamita, and not a bacteria. Secondary bacterial infections can occur in unsanitary conditions. It is related to poor diet, especially lacking vitamin C, and poor water conditions. Many fish have latent infections of the prtozoan, and when stressed, the populaion of bugs explodes. Healthy fish that are not otherwise stressed, manage finely with a few bugs in their gut.

Constant reccurances of HITH should (or at least ought) to point out to the fish keeper that there is an enviromental problem in their tank, be it overcrowding, temp, pH, conductivity, bioload and/or a nutritional problem.

I have raised many Discus without carbon in my tanks. If I do use it, I never use it for more than 24 hours.
