Pet Bass and 3 white spots

Hello all my pet bass has 3 white spots about 3 inches apart down its lateral line. They are on both sides. It looks like just the scale is missing. On on its head there is some whiteness but he has always had it. Just the white spots concern me. Also his fins are all good looking, just his pectoral fins look a little ripped up. I've had him for 4 years now and hes gone through stages like this, but these white spots have been going on for a while now. Can anyone help me with this?

Not sure about the spots on his sides, but it looks like you have a water quality problem,(definately not a healthy looking fish) by the white on its head and the small craters on its face. Possibly HITH or some type of fungus.What size is he?How often are you doing water changes ?and what size tank is he in?. They produce alot of waste and without excessive filtration and 50% water changes weekly or bi monthly, you will get all kinds of problems (fungus, parasites , etc). A 4 year old LMB should be in nothing smaller than a 125 - 150 gallon tank, if he is well thats your problem. A fish that is stunted by to small of an enclosure is much more prone to infection/disease. Frequent water changes and proper sized aquarium/filtration should turn him around.
