Please help! What is wrong with my betta???

Erythromycin is not the correct antibiotic for Columnaris. If this was Columnaris which I don't think it is you would need a different combination of antibiotics to cure it. This looks fungal to me I would use API Fungus Cure a combination of Malachite Green and Acriflavin. Do a 50% water change wait 30 mins after adding water conditioner or declor and add to tank as per package instructions.
 
Testing your water is always a good idea.

In addition there are some meds that are affected by certian water parameters and either work less effectivly or wouln't work at all or can in some instances be dagerous. Some species of fish can't tolerate some meds that work great for others. Columnaris is an aerobic gram-negative bacteria where erythromycin is best used against gram-positive bacteria such as Streptococcus and some types of eye Infections. So the use of the correct meds in the correct conditions is an important part of finding an effective treatment.
 
Thanks. The betta wouldn't eat at all until the erythromycin treatments. There is no way I can test for disease in my water, right?

No, no way to test for that. Try the fungus cure.
