Pleco with fin rot and a parasite infestation!!

Hello MFK community!! I'm surprised I'm here talking about my pleco out of everyone having a problem ( considering I'm constantly frantic about my bichir even when there's nothing wrong with lol.. )
I'm afraid he might have a mild case of fin rot I've never seen his sailfin so see through!! It started out as a small injury his sailfin (probably from trying to squeeze himself into a tight spot) so upon notice, I had my boyfriend vacuum the substrate and do a 20% water change to hopefully prevent any infections. When we did the water change he noticed that we have a flatworm infestation!! We've never had worms before and if we did we never noticed. I'm just worried about the impact that the flatworms might have in the tank and his injury. I've read that they're harmless but should I try to get rid of them? Also, any injuries he previously has had usually healed up within a few days. This one particularly worries me though due to his fin being so clear! And I feel like it's getting more see through by the day!!

Here's some info on the tank itself :
Recently switched from a 75gal to 120 gal / 120 gal tank has been set up for about 2 months
freshwater tank / no salt added / about 6 live plants
API Filstar XP Filter / filter has been set up for probably 7-8 months
Tankmates : Sailfin Pleco 8", Senegal Bichir 8", Striped Catfish 5", Pictus Catfish 5"
I'm not sure about the parameters of the tank.. I tend to leave that stuff up to my boyfriend since I'm only 5'3" and the tank + stand is twice my size, haha but I'll ask him to check once he's home.
The water is always clear and all the fish are acting normal. No one else seems to have any health issues. I'm not sure what's the best issue to take on this situation so any advice would be appreciated.
