Ray head swollen

Yes my p14 looked just like that.

It was shortly after getting shipped to me. I did everything! Talked to several biologist and, kept water as clean as possible, daily water changes, isolation, and treated for gill flukes.
Eventually they made full recovery!! Good luck!
I didn't have a flag tail or pleco btw

I had one of my marble pups do something like this never figured out what happened so just tried to keep clean water this ray did not make it unfortunatelyrays 119.jpg

This is exactly the same symptoms John. Its not just the head that's swollen its the whole body, in my experience like I said before it doesn't usually end well.
 
I have seen this many years ago. To me it looks like a retention of body fluids caused by internal bacteria (Dropsy). My guess is it probably won't get better but if it was me I would raise the temp to 28 deg C and add salt, nothing to lose, I have had some success with other fish by doing this.
