Really bad looking eye!!

Sounds to me like you have had a fight and his eye was just physically torn out.
Whats in the tank with him?
If this has happened before like you said I bet you have a bully on your hands!
Did you ever find the other missing fish? if so did you look for this one where you found the others!
seriously check your tanks decorations (Take them out and look!)

Here are some of the tanks dimensions and names/pictures of the inhabitants

1 x Yellow Lab
2 x Small Pleko
2 x Large Parrots 6 - 7inches (These are the dominant fish in the tank, very territorial and not really ever challenged. If any one was to be a bully i would point at these two first)

The tank dimensions are 1.9m x 1.5m x 0.6m, the slates are leant against the back wall.

I am afraid i don't know the slightest thing about the type of filters or testers, the Tank was already in my property when i purchased it. The tank was desinged, built and is maintained by Aqautic Design Centres of London, who as far as i can tell are quite reputable. Some on here may be able to give some more info on the company.

The new layout, design and inhabitants have been in since Sep '08

id say it was aggression. you should be doing waterchanges more often than once every 3 weeks. i do a weekly waterchange of 50 percent on my tanks. i keep africans and SA/CA's (NOT TOGETHER lol).

its a very pretty tank-i love the setup, but you need to create some cave like spots for the africans. the tall rocks will block line of views during fights and periods of aggression, but the less dominant or hearty fish need hiding spots from the bullies.. i would throw some small flower pots behind those rocks and a few in between..
