Sorry for you loss. Do you think he could have jumped and hit his head so hard that it killed him? That's the only thing I can think of. Condolences, I am looking at getting one of these fish and I know how hard they are to get.

Thanks everyone for the condolences. I'm suspecting a major hit on the heavy glass top, too. The water still tests out as being OK. I put some minnows in the tank to see if some toxin had somehow been introduced that wouldn't show on the test kits. The minnows are fine.
I'm checking into making a custom top out of 1/2" netting sewn to thin lexan. The netting will have a draw cord sewn around the edge of the netting to be able to lock it to the top of the tank (under the lip of the plastic framing). This should give me the elasticity to avoid a jumping injury while the lexan keeps evaporation to a minimum. If the netting doesn't 'wick' water to form a constant drip, it should work just fine.
