How fast is the fish breathing? Is it swimming into the powerhead?
Looks to me like late ich.
Luckily, ich and velvet can both be treated with copper. All fish should be treated in a separate quarantine tank, the clown may not make it but the others can still be saved.
The tank should remain fishless for 45 days.

Hard to make out but looks like ich to me.
