Depending on the tank size a good sized sponge filter or two will suffice. Just dont skimp on the air pump. AC hobs are great too.
The lfs gave you pretty good advice with the epsom salt. It acts as a laxative and will help the fish expel trapped air and blockages.
It does look like gravel to me.
