Severum not eating/lethargic

Finally time for a positive update!

Mouth still looks pretty bad (but doesn't seem to be getting worse anymore), two days ago I was on the verge of buying antibiotics, but I wanted to give him one more chance at eating. I went to feed the tank, and he showed a ton of interest in food for the first time in a while, but didn't take any. I then fed bloodworms and he aggressively went after and ate a few! Yesterday he again ate bloodworms, so while its not the most nutritional meal, at least he's getting something in him everyday again! He also seems to have more energy again and isn't hiding a bunch anymore.

This is giving me hope that clean water and salt is working and antibiotics aren't necessary! If he stops eating bloodworms and gets more lethargic again, I'll immediately treat the tank, so fingers crossed he continues to progress in the right direction!

Update:

Im very very frustrated right now. He has stopped eating Bloodworms but visually his mouth looks better:

There is a little whitening on the other side of his mouth now too. I dont really know what else to do besides antibiotics.

any recommendations between Kanaplex or Erythromycin for trying to treat this?

I’d try the kanaplex. Try refeeding the bloodworms.
 
Agree on trying the Kanaplex. Good for "several bacterial and fungal fish diseases." I used it to clear up some stuff on a wild severum I bought a week or two ago (I had some around). Not supposed to bother an established filter, which is true in my experience. You can get it in 5 gram bottles, not expensive at that size. If my math is correct you'd need two of them to treat a 125 gal tank up to the 3 recommended doses.
 
Yesterday I got him to eat bloodworms again! Also when I fed the pellets, he really wanted to grab some and would get right up to them, but backed off.

His lip seems to be almost 100% healed, but he clearly has an issue still in his mouth, you can tell it hurts him even to eat the bloodworms.

Also as for the Kanaplex, I bought the big 80 gram bottle the other day and it should be here on Monday. If I have extra, then so be it! Doesn't seem like the worst stuff to have lying around.

Welp, Kanaplex treatment is done.

The exterior of his mouth looks fully healed, but he still seems to be in a lot of pain when eating and is only going after bloodworms. It looks like there is still a red mass in his mouth, but its very difficult to get a good view of.

I'm really not sure if he is going to make it, just gotta hope time and good water helps him recover. At least he's getting some bloodworms in him every 3 days or so....
 
I gotta say, this dude is a champ so far. It looks almost completely healed on the outside, but the inside still looks red and torn up. It’s now been almost 3 months of not eating any pellets or flakes, and just bloodworms every couple days, and he seems to finally be turning a corner.

ever since the Kanaplex treatment finished he has been begging me for food everyday, but won’t eat it once it hits the tank (except for bloodworms, which he has slowly been eating more of each time).

Today I watched him actually take a few flakes from the surface for the first time since early April. I fed bloodworms tonight and he ate them more aggressively than he has in months.

We aren’t out of the woods yet, but compared to a few weeks ago when I was pretty sure he was going to be a goner eventually, I think it’s a 50/50 chance now.
 
Think it’s only been eating bloodworms from earlier posts. This might be a good chance to get it on a pellet of some kind.

I know before I feed my fish I soak my pellets in water for about 5 minutes to soften them up.
I do it in the theory that it will help bloat, as there is no air they are ingesting, I also buy sinking pellets for that reason. But I think It might help with your problem too
