Sick Oscar, please help

Just want to get your input. I have 1 oscar in 40 gallon tank, I have him for 20 months now, since baby and now he is about 5-6" long.
He refused to eat for the past 6-8 wks, he had "pop eye" on his left eye but now it seems ok. He barely swim around, and some time he float side ways. He has "hole in the head" and his gill curl a little.

Any suggestion? Water Quality is good, I do Water Change every week, about 15%. I always feed him Hikari Cichlid Pellet.
 
yea rectify tank conditions and use some salt and if he doesnt seem better after a couple days ur gonna have to euthanize
 
well dont forget to clean the gravel...just changing water wont do it...there really messy...but hardy too...try cleaning the gravel good and like the guy above me said...try some salt...he might just be really stressed out
 
Wow, yeah, sounds like really bad water conditions. An Oscar doesn't really belong in a 40G tank, in addition to getting big they are super messy for their size, on a 40G tank with an oscar I would be doing 2 50% water changes a week with a good gravel vac each time. And I would be sure to over-filter a bunch as well. How do you know the water quality is "good"? Have you tested your parameters?
