Sick Oscar, please help

What are you using for filtration?
How often do you feed the fish?
Do you remove leftovers or do you leave them in the tank?
What are your water parameters: ammonia, nitrite & ph?
 
first of all 40 gallons is not enough for an O. i dont care what anyone says about it being enough, IMO that is what is making this fish sick. you need to feed him more than hikari gold, he will need white fish...prawns...lots of different food in his diet. and you need to get a test kit. no good just saying your water param is good. its clearly not, if not your O would not be in such bad health. you need to move him to either a minimum 55gal..but by the sounds of it..he doesnt seem too successful in chances of recovery.
 
i agree with neervana. the tank is too small and 15% is just not enough especially with his size. like every one else said try some salt. if he gets better feed him other stuff too, that will help with the hole in the head. but absolutely no feeders.. try earthworms, waxworms, peas, mealworms, crickets.. in addition to his normal staple pellet
 
that fish should also be well over 5-6 inchs if youve had it for 20 months, hes obviously stunted(most likely due to poor water quality.
 
the Oscar should be at lease 10 inch to 12 inch already. bigger filter and more water changes and salt AND A bigger tank.

Exactly an oscar should be able to reach 6" in about 6 months with good water quality and a good diet. Your fish is in rough shape.
 
I hate to say it too even though i know everyone's flaming on u! I got four oscars when they were one inch only! and that wuz two months ago, they're already 5 going onto 6 inch! i change their water every few days and have to vaccuum all the krill bits they spit back out before i sleep! it's a hastle, but that's what u need to do to take care of the oscars!
And u gotta feed those guys some krills.. they need all sorts of food for a steady diet!
