I'm so sorry for your loss. He might hit his head on the lid... I had a few that done that, whether from striking for food or something on top of the tank, or got spook by other fish... They like to go up... up... and away without hitting the break

Yeah thats what I'm guessing too, he used to strike hard when feeding and even though there is a good 3 inches between the top of the water and the lid, he would often hit it.
