I don't think it's much of a surprise that Rick will survive. I've now seen a video of Negan talking to Rick after the killing. Rick has blood spattered on one side of his face which gives a clue on which side of Rick the killing took place(unless Negan smeared it on him). One would think the victim would be somewhat close too.

I've never read the comics but apparently he lost his hand in one,that certainly makes sense here.

Not sure...some of the producers don't want it to happen. 'Rick' himself has said he's for it. That scene sure is suggesting it could happen.
