Are you buffering your water to adjust pH. This is one other source of phosphate depending on the product used.

Vladfloroff, you brought up a good point. In fact I had a customer last week with this problem. He was using both proper ph7 and Diuscus Buffer to try to get a targeted ph level. He had no idea that these are phosphate based and though he performs large water changes frequently, his water was pea green and his phosphates were off the chart.
