What is this stuff, it keeps reappearing on my tank glass.. It was gone for awhile after I did a medication treatment in my tank a few weeks ago and now it's back.. I looked up and heard of detritus worms but this is way smaller...

Planaria? You seem to have an infestation though, without knowing any details I would say you probably feed too much.
maybe but I do a water change in my tank everyday... At least one bucket I change out
 
Ya that's also only one small section of my glass... It's like that across the entire top of the tank all across
 
When's the last time you cleaned the filter? Or gravel vacced?
 
Yesterday.. I just added a brand new filter on my tank about a week ago.. Also I have Sand as substrate so I really only go over the top of it... Like I said I do that Bout once a day and my main filter that has been on my tank I do maintaince on it about once a month

How big is the tank in question?

A 5 gallon bucket water change for a 20gal tank is adequate, but a 5 gallon bucket water change on a 75 is insignificant.
 
Its a 29g, also a I do a 25% water change once a week
 
Definitely appears to be planaria. They are harmless to your fish but an indicator your tank needs better filtration/water changes
 
I thought that detritus was harmless to fish not planaria and also that planaria had nothing to do with your tank
