Id say this is something due to your lack of wc and extremely high nitrate. 190ppm is way to high. I panic when my nitrate goes above 20ppm.

I have always obsessed over clean water, a healthy obsession I think in this hobby. Early on I used nitrates as a measuring stick for overall nastiness, higher nitrates in a fish tank generally means higher levels of dissolved organic compounds, higher levels of bacteria, and over time a reduction in minerals, often accompanied with the overall acidic build up, a potential lowering of pH.

Over filtration won't help with any of the above.

Yeah what a bummer! Definitely need to stick to a very strict wc schedule. I went from every week to 2 weeks and ended up at once a month which caused this issue.
 
I have tanks the same size as yours, as an example, in the one 125 gallon I have a single 13-14" male midas, filtered by 3 AC 500's (110), and that tank gets an 80-90% water change weekly. If I have extra time on my hands, I might get another 30-40% change in mid week, but I never miss a weekly 80-90% WC. Here he is at 7+ yrs of age.

That's a prestine fella and tank. Since he's so big if you do a 90% wc does he end up on his side for a period of time?
