I run my Xps with the foam on the bottom, biomedia in the middle and polyfill on top and the water is chrystal clear even in a bit of an overstocked tank. If that doesnt help perhaps invest in a magnum with a micron cartrdige. It just sounds to me like some free floating particles that are to small to be picked up by your current filtration. Bacteria blooms dont last for 5 weeks straight...and I doubt its an algae bloom since youve done all your take covering and such. IMO its just not enough mechanical filtration.
