Water quality issue?

I'll be the first to admit that i fell behind on my water change schedule with this tank. I have had the. For about a year now and have gradually become more relaxed with their routine as they have proved to be the hardiest loaches i have ever kept. I am a complete loach novice. I love modesta but cannot keep them alive no matter what i do. I impulse bought one of these royals and fell in love with it so i coughed up the money and acquired 2 more. They have been doing really well in this tank for about 14 months all together. Until now. It at first started as white patches on their skin and yesterday i noticed one of them with the huge open wounds in the picture. I thought it was over for him the wounds are enormous. They had some white fuzzy patches on them at first but i immediately did a huge water change and added a bit of salt to the water. I have noticed the white patches once before about 3 months after getting the original one but it went away not long after noticing it. They hide a lot so i dont always see them. Do you test your water parameters?
Do you have other fish in there with them?

Would suggest a liquid test kit and get some numbers done ASAP.

Add some current to the tank and do a water change daily with a dechlorinator till test results can be obtained. What’s ur filtration?

I am using a old fluval internal filter. Works really well. Mainly for flow/current but it does mechanical filtration pretty well also. Will get a new test kit after work
