it happens.. i did a WC on my gf nice planted community tank a while ago... even added prime... super funky "no one knows why" tapwater managed to kill off 3 congo tetras, 1 oto, and put one heck of a hurting on my L239 (fancy pleco)... i spent like 4 hours holding my pleco infront of a powerhead in a different tank trying to keep him breathing... he made it but he ended up with a MASSIVE case of ich (my first ich outbreak EVER)...

I found that even when adding prime during water changes i lost fish so i decided to buy a 220l water drum. I would fill the drum a day before a water change, add prime and salt, then on the day of the water change i would chuck in a few buckets of hot boiling water to heat the water up, add prime and let it sit for a while.
A pump then pumps all the water into the tank after a siphon. Haven't lost a fish after a water change since and it makes the WC process a while lot easier.
 
wow that sucks, I know my water is laden with chloramine. That is like instant death for fish unless you dose 2X with prime for the whole tank volume.
 
Wow i hardly add any thing to my water to remove chlorine and if i let the water sit for 10-15 mins with an air stone in it i don't have to do any thing
 
also, if you suspect you have chloramine one way to test is to do a ammonia test on your tap water. In most cases it will show a high reading. in my case it was 4+ ppm straight out of the tap. I called the water company and they said no chloramine,,,,well dont beleive that either. trust your test kit (API) and follow your ammonia readings
