Do you condition your water before putting in or just fill and add chems? 50% twice a week could've stressed it too far, and then the rest picked it off afterwards.

No I don't - I add prime and cichlid salt while it's filling with new water. How would one go about conditioning the water before putting it in? Isn't that a lot more work?

Using prime and your salts is conditioning the water.
Looks like he got wasted it happens.

That's how i do it too, but some fish can stress out and croak with that much new untreated water a week. Just throwin out a guess.. Either way, the tankmates just had their way with it afterwards, sheet happens.
