What is happening to my plants??

Here is a picture of my tank. Its not too good because I took it with my phone, but I thought it may help to see what other plants I have. The sword on the left was added yesterday and it already is showing holes in the leaves a well. Please help I'm losing all my swords! :( The second picture is a close-up of the leaves.

Ok, so the swords are fairly new. Where'd you get them, and were they sold to you as a submerged plant? If they were grown partially submerged then the initial leaf rot is expected. The plant will grow new leaves rather quickly.

This would explain the ammonia spike. Rotting leaves also release ammonia, as well as leach nutrients back into the tank raising the possibility of algae.
 
All of the swords have been in there for a year. I added 1 new one the other day as an experiment to see if it would have the same symptoms and it does.
