I have been in the hobby for quite some time and have been pretty fortunate to only come across a few common illness with my fish but this one is leaving me stumped as the closest thing I can relate this to is Hex. I had 6 good size Temporalis at about 9 inch. I recently lost 2 about 1.5 months ago to this nasty disease. I didn't take pictures of it unfortunately but it left the 2 fish in a very zombie like state with big holes and carvings around the head and eye and tore up its anus.
The tank is very large and I couldn't get meds for the whole tank and tried with Salt, Epsom salt and Prazi. It seemed to have gone away after I lost the two. There is about 30 fish in this tank and only three chocolates were effected and the third with the most minor symptoms returned to normal.

Well it now has returned again and three are showing symptoms again.
It starts at the anus and looks like you took a drill bit to it. White feces follows then the rear sides of the body by the base of the tail gets a white velvet coating as well as the head . Following this small white bumps appear on the head that then turn to large white spore like tubes that stick out about a 1/4 inch that then leave massive torn up holes just before the fish is completely lifeless.

No other fish are effected , only the chocolates.

Some photos I tried to take are attached.

As you can see in the one photo the little white specs will eventually turn to large spores . It's a very groosom sight.

Sorry to hear this Mike, I'm totally stumped as to what this may be. Hopefully someone else will have some info that can help.

Yeah, I’ve seen that once when my nephew bought an albino Heckelii from a guy that kept them in ponds. It was absolutely nasty stuff and had similar symptoms with several white tubercules that seems to grow out from the sides near the abdomen, like the pic of your chocolate. Thought it was worms crawling out through the body for a moment...so gross. Tried metro, prazi then switched to antibiotics when the sores on the sides became infected. Tried just tons of fresh water changes, salt, malachite green, etc.

Would look better then come back again until it finally kicked the bucket. Wish I could steer you in some direction but we had no luck with that heckeli and never figured it out. Did some research that was inconclusive but remember thinking I need to look into body flukes. Hope you get this sorted
