I have 2 22'' koi that I have to man handle, and I knew what I was getting into before I bought the fish.

If you don't want to handle it to get this off, maybe it's not the right fish for you?

Not trying to sound rude, but there's nothing we really can tell you without a scrape and scope for a definite diagnosis.

Except that I have 5 of them that I've had since they were 2 inches!
(Note the Sarcasm).
If you read the thread, I'm looking for someone who has experience with this particular problem. Once diagnosed,I would do what ever it took.
I just am not willing to start digging at some unknown thing to stress out the fish further. Had you any experience with these particular fish, then you would understand how stressed they can get.
The first rule of medicine is "Do No Harm".
So in my opinion, I think it irresponsible to attempt such a feat without knowing the source of the problem and how to properly eradicate it.

it is a parasite. try to use parasite clear from jungle it has worked for me on almost every parasite problem ive had. from internal to external....
