Hard to say for me what it is so I'd catch it and use a cotton Q-tip and disinfect the wound with a mild antiseptic - hydrogen peroxide, potassium permanganate, iodine, etc. - at least several times for several days to see if there is any effect.
Sure it will stress the fish, yet cichlids are tough.
