I say since its close to the mouth like that it was a fight. It could also have been startled and ran into a sharp object in the tank. Good thing its healing tho
 
If that's not a fungus, it's a nasty form of HITH. If untreated, his head will explode with 'cottage cheesy' white stuff and he will die soon afterwards. I've dealt with this in a few past fish (a fh, a GT, and a pacu). In the case of the fh, I treated with AP med to treat 'hexamitia'. The problem went into remission for months only to strike back with a vengeance. It's unsightly; therefore, once there is significant tissue damage and the fish stops eating, I'd euthanize.

My 2 cents -I've seen some pretty aggressive forms of ick and to me that's what it looks like. Starts out really fast - almost overnite. This forum of ick looks like spider webs engulfing the fish and it does spreads really fast. With that form of ick - salt and higher temps wasn't fast enuff. Quick ick no dice - mardel cletted cooper is the only cure that has worked for me. If that is what it is you don't have a lot of time to scratch your head. Posting in the disease forum is a really good idea.
