What's the most grusome death one of your fish has suffered?

During my battle with Ich, i killed my two smallest clown loaches with a heater/circulator pump. First one swam up the intake hose and to make sure it didn't happen again, i covered the intake hose with thin sponge filter mesh held on with a hose clamp. Unfortunately there wasn't enough flow, so I started cutting little holes and slits in the material to increase the turnover. He also got sucked up and I found him in the pump... I was so upset that i was trying so hard to help them but they kept dying, and in those two's case directly because of my mistakes.
 
I had an Oscar eat himself to death...The last time I got a treat of feeder fish.. I didn't know that they would or could kill themselves like that.. I felt so bad because it was my fault.
 
I was cleaning the prefilters and intake tube on my powerheads, i made the mistake of leaving it run, and one of my kuli loaches swam into it and got decapitaited. i was in the back room when it happened. all i heard was this loud buzzing noise, came out to find his body in the powerhead and his head floating around the tank. Now i always shut them off when cleaning.
 
had a very large male JD 8 yrs old 10 ' for about 5 months super healthy and paired up with my jag, was waiting for amazing looking babies.. but i left town for a week, someone forgot how much to feed my fish, fed about 60 dollars worth of pellets flakes and sinking pellets in one week..thanks uncle...:irked: cleaned my tank, then notcied the next few days he developed pop eye and bloat. Qt'd him in a 50 gallon, 3 days into qt, he decided to blast thru my plexi lid and i woke up found the lid on the ground...and dempsey was nowehere to be found:S
20 minutes later..after still looking, he jumped over my tv, landed in the corner about 6 ft from my tank. poor guy couldnt take it anymore :(
 
Well my dither bala shark just died my midas finally got him lol bala darted hit the glass got stunned and bam the midas goes for the eyes and sucked them out lol owell nevercared much for the shark though just a dither

My 2 year old daughter put a huge piece of pizza in the sump of my 240 gallon aquarium and within about 5 hours my aquarium went solid brown... After removing all peacock bass and oscars and doing a 100% water change and cleaning everything in the tank (5 hours later) everything still lived lesson learned on having a open top sump... Switched to closed sump system and child locks on stand!

I had a tank with 3 redtail catfish (my second batch) and 3 shovel nose catfish. To this day I am not sure what happened but the entire tank bottomed out and they all died. The median was changed the week before, the heater was working fine, they ate the night before. I wake up the next morning the water is foggy and they were all floating dead.
