40 Nitrates sounds kinda high to me...have you checked the ammonia? When was your last water change? I would say keep his water in good condition, keep the temp up. Most likely it will clear up, I have no idea what it is and wont act like I do...but I know excelent water is a key factor in healthy fish.
He may take a while to accept it but the pellets are a good food for him, I would continue to try and feed him that, then start adding other things like blood worms, beefheart, shrimp, some veggies.
