Whats wrong with him?

I have a juvy tiger oscar about 5inches in a 100g 48x2x2 tank on his own, Ammonia - 0, Nitrite - 0, Nitrate - 5
He lies there on the sand and breaths heavy with his mouth open and doesnt eat, he aint come up to eat in about 2 weeks.
He gets fed 4 tetra xl cichlid sticks everyother day 4 days a week, friday he has bloodworm, saturday he gets green peas, sunday he I dont feed him.
He swims around every now and again, but just lies back down, slanted.
His mouth is constantly open.
Any idead?
 
I personally noticed with larger Oscars after they eat they lay on the bottom of the tank.

Oscars are kind of weird in the fish world. They're proof that fish do get depressed, there could be a lot of things going on here though. From internal parasites to not liking the tank (the way it's arranged, substrate, etc) to not liking the food being offered. A lot of variables, especially with Oscars. Have you see any poop from it? If so, was it brown or white and stringy?
 
The poo is white sometimes and brown sometimes, ive tried him with frozen bloodworm, live shrimp, no feeders though.
He refused all.
Tried him with an earthworm, he ignored it.
Re-arranged the tank a few times, he just lays there and let the food go past him.
His mouth is constantly open and he breaths slowly but heavily.
Any treatment I could put in the tank?

It could be internal parasites. Since it's not really eating you're going to need a medication the fish can absorb. Since it's the only fish in the tank, do you have a 10 gallon tank? It'd be cheaper to treat 10, than 100 gallons. I've always used the Tetra/Jungle Parasite Guard with great results.
