Where did you buy your Monster Tanks form?

I never dealt with them since I purchased my tank through Fishtanksdirect.com. My warranty says Sun Coast Aquariums. (Clearwater, Florida, according to doviiman) They don't seem to have a website. I found this note about them on another forum:
