It can happen with some wen fish especially during mating season. One of my orandas developed white spots around the eyes covered in wen, sign of breeding season.
If the white spots appeared solely on gill covers and front pectoral fins, definitely it indicates the fish is a male and ready to spawn anytime. This is a sign of sexual maturity. Very small goldfish that exhibit this sign could be stunted.
The white tubercles are often mistaken with ich. Ich often appears first on fins before it reaches the body so the distinction is quite there. The affected fish will also scratch badly as a result of the parasites trying to irritate the fish especially its gill tissues.
