Understand fish have sensory pits but that really does look like the start of hith.
Probably caused by having 3 oscars in a 55.
If you must confine those fish in 55 gallons I would up your water change regime to 90% twice a week. I don't think 30 to 50 % will cut it.

We understand very well when to criticise. You are another good case for criticism.
You were advised to part your flowerhorn and Oscar ages ago and you still have them together ripping each other apart in a small tank.
