what is your tap water testing at? we made the mistake of doing water changes with tap water here and then tested the tap water, and found that the tap water had 3 ppm ammonia in it.. so we were causing our own ammonia spike.

we also found that the tank in question had very low PH.. we added some crushed coral and the ammonia spikes stopped, the rotten smell went away and the tank is stable now.

Are you testing RIGHT after you do water changes? That's weird, it should at least change some.

Are you using prime or another ammonia neutralizer? The ammonia will still show up on the test kit but at least it won't be as stressful for the fish.

How are your nitrates and nitrites? How long has the tank been setup?

Few questions. First question is what type of test kit are you using? How old is it? Are you following the instructions of the test exactly?

Replacing filter media when your tank has ammonia is a bad idea. That will just prolong, if not completely restart, the cycle process.
