ya nitrate 0 is wrong.
That means your tank is less than 6 weeks old.
Cycled tanks probably have at least 10-50 ppm nitrate.

+1 your nitrates should never read 0. Make sure you're following the directions step by step. The API nitrate drip test is a little different than the rest. Make sure you're shaking the bottles as directed and waiting the amount of times directed
 
I do water changes every week in my 500. I add dechlorinator AFTER I am done filling the tank. I have never had an issue. That said,I have seen fish "freak out" and die suddenly. No obvious reason. I really wouldn't worry unless other fish do the same. Losing a fish now and then is just part of fish keeping. I have had certain fish that lived 5 years while other tank mates of the same species die after 6 months. Why? Who knows.....

Some master kits come with a test for NITRITE and not NITRATE, maybe you're testing nitrite instead of nitrate.
