Bloodworms aren’t good as a staple, and even if temporary, the fish can get addicted.
Try to get it eating pellets.

sounds good. Any simple ways to get her to eat the pellets? Would garlic guard help with making her want the food?
 
Yes, garlic can help with some fish. I have another trick for blood worm spoiled fish.

Presoak your pellets of choice in the juice of frozen bloodworms. Only feed once per day, in the AM when the fish is most hungry, and do not offer any other food. Eventually most fish will slowly but surely start nibbling on the food. Once it is eating like gangbusters slowly over several days wean off of the presoaked food, and convert to 100% pellet. I have trained juvenile fish that were raised exclusively on bloodworms (so super finicky), and had 100% success rate with this method. Good luck.

So as an update, I tried all the suggestions and nothing has helped so far.. been doing water changes every day and keeping nitrates less than 5ppm had fed her metroplex for a couple weeks and I can’t tell if it’s helping the hole in the head (looks slightly worse) but she won’t touch pellets no matter what brand/size. I soaked them with bloodworms and tried garlic as well. Any last ditch efforts? I don’t want her to die on me I feel horrible. Thanks for the help!!

Can try General cure to the water or a metronidazole dip along with medicated food if it’ll take it.

for how long? And she will only take metro plex when added with bloodworms. So should I keep feeding her bloodworms or keep trying to get her to eat pellets?

I’d keep treating for 14 days with the metroplex if it’s taking it. Mix with a regular pellet and keep doing till the appetite is better and poop is the color of food.

She will only take it if mixed with blood worms, should I feed that anyway?
