Is the bacteria actually caused by fish or the water? I never knew about this...

Bacteria is from the fish. In my case, I pulled out a 30" dead koi out of someones pond to dispose. My middle finger tip swelled up 2 days later which cleared on it's own in two weeks. Two weeks later, top of my hand developed a big sore larger than a quarter.
For pools, the doctor said it's bacteria from people living in the under treated water.

I'd say anything with decently strong venom (Rays, toadfish, etc.) can kill someone with a weak immune system and an allergy.
Electric eels... if it's a huge eel and a human with a heart problem, could happen.
Other than that, some fish might be able to injure someone pretty badly but never would they kill someone.

What fish would you say could seriously injure someone?

A sw lion fish can give you a very very painful sting through its venomous dorsal spines. It is possible for some people to have an allergic reaction to the venom, which comes with a host of potentially deadly complications resulting from anaphylactic shock. So the potential is there. I would also hate to fall into a pond with 4-5 hungry Giant Snakeheads...that might suck. And if you have any of the aquatic turtles, like the pig nose turtle...they recommend you wear elbow high rubber gloves when working in the tank because although fish bacteria can cause nasty infections, the bacteria found in these turtle's waste is of a much nastier variety and can be lethal if an open wound or cut is exposed.

lion fish, other venomous marine fish are most likely to have caused death imo, allergic reaction then death. other than that the toothy heavy hitters such as a big black rhom, gaft, aimara wolf, moray or giant snakehead severing an artery during maintenance i guess is a possibility... die after 6 min if the main artery in your arm is severed right? of course theres that orca at sea world, does that count as an aquarium fish!-)
EDIT /\/\/\ beat me too it lol
