I don't think that viewers would really "care" about watching a gloved hand with a piranha dangling from it, doing no damage. I mean, what IS the message here? A bluefish (a common fisherman's quarry) does a lot worse than a piranha, out of water. There is a reason for the nickname "Choppers" these fish carry.

I think what you described asks for something more raw, something more real, and if your guy really does want to be extreme, it needs to be hard skin to skin action. So, in other words, it has to be something that won't do SERIOUS damage.

A large lungfish bite is said to be extremely painful, but he won't get his finger/hand ripped apart. (I think!) Their jaws are ULTRA powerful. How powerful? That's for your guy to find out - which will give TV what they want.

Basically, lungfish are this "huge" sausage shaped fish with leglike fins - Looks like a prehistoric monster, and it is, in fact just that. It's one of those things you never see on TV. It would be extremely freakish and interesting, and NEW, and unique.

And at 2 to 2 1/2 feet (and as big around as a man's arm) it does not invoke "fear" so it has a very effective element of surprise.

Look at the photo in my avatar for a piebald albino example of a lungfish, and below is a photo of my "big guy" About 20 inches, or a bit more. This is the natural color of an African lungfish. I am also including a photo of the piebald so you can see it better than in the avatar.

Let him get whacked by a larger mantis shrimp!!!! Many people dont know how powerful these awesome crustaceans are!!! Also I have had a chunk removed by an octopus and that is a painful experience. Maybe get a giant Tridacna clam to close up on his hand or something!!

a large lungfish bite isn't that bad. Mine was 26" and he bit me a few times. It doesn't even break the skin. It more or less smashes and scrapes like getting your finger closed in a door or something equivalent. Never been stung by one but I think a stonefish or lionfish would be good to show. Most people think they are all deadly. Here's my favorite picture of the lungfish I had:
