You should send the pictures to Fluval. I'm not blameing them, it's hard to imagine something like this happening. If they realize it's possible for fish to get trapped like this, they might redesign the outlet to prevent it in future systems.

I had goldfish to cycle one of my tanks with an fx5 and they dissappeared. When I went to clean the filter for the first time there were some really fat happy goldfish living in the bottom of it still alive. Strange huh?
