Yes, do a 50% water change just before the next dose of Kanaplex.
I saw that you gave the full dose, and it is a good idea to be cautious with medications you aren't familiar with. It's just nice to know how the drug works, and why the instructions are what they are.

A bit more info about the drug and what it targets: As mentioned before, many fish pathogens are gram negative bacteria. Kanamycin specifically targets gram negative aerobic bacteria. Unfortunately, the nitrifying bacteria that we depend on to convert ammonia to nitrite, and then nitrite to nitrate are gram negative aerobes as well, making them potential targets of kanamycin.
According to SeaChem, Kanaplex "does not adversely affect the filter bed." I'm not saying SeaChem is wrong, but it would be wise to monitor your water parameters regularly during the Kanaplex treatment.

I've used Seachem Kanaplex at the recommended dosage (full strength) without any major bio bacteria die off, but that could be due to the fact that Kanaplex is only comprised of 40% kanamycin. (and 60% potassium sulfate) Regardless, it's always a good idea to keep an eye on ammonia/nitrite levels when using any type of medication.
 
Okay i just tested everything. All test normal for my tank. 0 amm o nitrite and a lil lighter than 20 ppm nitrate. ph is around 7.7 to 7.8. So if the first treatment wasnt effictive. Should i do a small wc today than another one tom before the next treatment? This way there will be less epsom in the water. Aro vent is definetley not as swollen. He looks like he wants a water change! Haha or is that me? The rays belly looks less painful. Not anywhere as red as it was. It looks like its healing as well. Just not as fast as i wanted. It didnt spread in size at all. That doesnt sound like bacteria does it? Shoul di be adding a kanaflex solution inside the food as well?

Aro wouldnt eat this morning. Maybe hes full on ray poop bc i cant find any! Something tellm me his poop eating is half his problem
 
I was thinking of adding some larger pebble type gravel to make the poop harder to find than the bare bottom.

Also, does anyone think buying a high quality uv sterlizer would help? Any experience with these units?

Swelling on the vent has gone down a bit and the color has changed from a deep red to a pink but this afternoon I found a weird change. I thought that he was pooping but than upon closer inspection I noticed it was red. Its hard to tell if its sticking out of his vent or a swelling off of the outside. From pictures ive seen it doesnt look like a prolapse as its very narrow..like a poop..but Im obviously concerned. He is eating still and acting otherwise normal. Any ideas?
 
For now just focus on completing the Kanaplex treatment.
Don't expect any overnight miracles, these meds need a chance to work.

I wouldn't add anything to the tank for now, pebbles, UV's, or otherwise.

Have you done a WC since the first treatment? If you did the 50% WC just before the second treatment, then you shouldn't need to do any more extra WCs. Kanaplex can be added to food, but just adding it to the tank works as well. The drug is absorbed by the fish either way.

I agree with R.D. Just focus on the treatment plan you have going for now.
