Your filter is not strong enough and cannot keep up with the bio load. What do you do with your filter media while you rinse the pads?
I only have this problem when cleaning the filter. The filter could sit for months without a fish lose. Since I got rid of the filter I'm running a Fluval FX6 and a Fluval 407.
I just rinse the Phosphate pads in Tank water and the rest of the filter just sits out for a few minutes.

So the bio media is left in tank water correct? What kind of media are you using & how much? Do you drain the dirty water out of the canister & fill it with tank water?

Yes I drain the Filter Water and fill up the canister with Tank water. No the Filter media sits in The empty filter for like 5 minutes.
 
I think the Problem is with the filter. I could not get it primed sometimes and it Would sit for a hour or more with water, but the water did not have circulation. I think with the new filter I will not have fish lose. Much easier to prime the new filter and I have much more filtration now. Just my opinion.
 
There's your problem.. your killing your beneficial bacteria. It should never be exposed to air for more than a few seconds. 5 minutes will lead to a mini cycle which you are dealing with.
Let me clean the new filter this way for now on and let’s see what happens. like I said it only happens when I clean that filter.
