It depends. If the fish has the will to resist it and continue thriving despite it, then it's not fatal otherwise it can be part of the burden of most sick hosts thus killing them quicker than the ones with the strong will to survive.
Antibiotics can affect your nitrogen cycle, yes. You might want to make sure you have backup filter media seedings in case your antibiotics manage to disrupt the nitrogen cycle successfully.
