I had planaria (they are a nematode) in my 125, and all I had in it was my RTC, at the time. He ate every piece of fish and every pellet that went into that tank. I have heard they come from overfeeding, too, but I can't believe it, now that I've had that happen. There's no way there's leftover food with a red-tail!
planaria are most definitely not nematodes the are platyhelminthes and from what the op has stated they are not planaria they are definitely some sort of round worm(nematode)
 
red-tailed rasbora love to eat these worms i had hundreds of them in my piranha tank now with a dozen or so rasbora very few worms see 1 or 2 at water change now 8'' piranha dont seem to bother with 1'' fish but might have to replace a few once in a while
