Maybe its a Goby... isn't there a Goby that lives in the ass of Sea Cucumbers?
 
theres species of fish that are attracted to urine and they get up into the hosts body this way... he did not push a fish up a penis.
 
Candiru catfish have been known for incidents like this assuming of course, the fish that he was handling is indeed a candiru. But isn't this in line already indicating it is a candiru catfish? "When he was passing urine, the fish slipped from his hand and entered his urethra and then he developed all these symptoms."

So he was keeping that species as a pet? Why wold he carry it in his hand to go to the bathroom? Sounds fishy...
 
While candiru are normally small fish – an inch or two at most - the catfish removed in this case wasn't a little one. It measured an eye-watering six inches in length and half an inch wide! Given the choice, I think I'd personally prefer to be stung by something instead...

To me, the fish he handled is indeed a candiru catfish. Only a few species are capable of such action and candiru has been most responsible for cases like this.

Considering candirus came from Amazon basin, I have to admit I am also intrigued why someone would keep a dangerous fish as a pet. I don't remember if candirus are indeed sold around the trade but considering their fatal attacks, I hope not.
