looks much better, rather than feed him mealworms though give him a pinky so he starts adding some plumpness. Cover some of that top up like mentioned earlier and offer him a nice hollow log or a piece of flat wood so he can use as a shelter.
I agree. Try a pinkie, or some more "crawly" bugs like crickets. Another thing you can try is feeding him at night. I had a water monitor that I got in really bad shape, and it wouldn't eat. I kept him in a tank with a glass top over 3/4 of it and when he finally started taking food the first was p/k pinkies or fuzzies that I put in overnight. Lots of heat, lots of humidity, and he perked right up.
Something you might think about re: substrate...I like to mix in a little cypress mulch with my dirt, it helps keep it from packing. If you notice the dirt getting packed try that.
