Pull him out or better yet, do a 50%+ water change!!!!!!!!
Then add salt, and on your filter on the outlet of the water, take off the nozzle so the water breaks the surface. It will create air bubbles

Sweet, try to get him as much aeration as possible and do a big water change. You didnt or are not in the process of starting up your tank are you?

I agree with adding some salt at this point. Melafix probably will do very little at this point as it's something internal. Add the salt slowly over the next couple of days until it reaches 1 tbsp per 5 gals. Don't do it all at once or you risk osmotic shock. Do daily water changes. The first water change which you have probably done already should be around 50%, then do 25% waterchanges daily, or twice daily depending on your preference. I've done this for a dying lima before that acted like your's and somehow it pulled through and I still have it today. Good luck. I'd hate it if my jurenese were to die. Hopefully this will help some. Just make sure that you have plenty of dissolved oxygen as already mentioned. That's really key, and the salt will help the gills work more efficiently while he's sick.
