Seeing as it sounds like you are still in the weeds with this fish, if you decide to also treat with additional metronidazole to fight what is almost certain to be a case of Spironucleus, some things to keep in mind.

In fish, an excessive use of metronidazole can damage kidneys and other internal organs.(Bassleer, 1983) I am not certain as to how he defined, excessive?



Dr. Edward J Noga suggests:

1. Bath

a. Add 19mg/gallon and treat for 3 hours. Repeat every other day for 3 treatments.

2. Prolonged Immersion

a. Add 25mg/gallon and treat once daily for a total of 3 times.
b. Add 95mg/gallon and treat every other day for three days.

3. via feed

25 mg/Kg of body weight/day for 5 to 10 days, or 100 mg/kg of body weight for 3 days. Retreat once if required.

His lab at NC State University specializes in the study of infectious diseases of finfish and shellfish. His book, Fish Disease: Diagnosis and Treatment is the go to "health" book for every serious fish keeper and aquatic DVM on the planet.
 
Is this definitely hith and not just sensory pits?
I could be wrong but the pits are symmetrical and in exactly the same place as my old males.

Short of weighing the fish and getting an exact number to go off of, ballpark it. If he is already hesitant in eating metro laced food, I doubt that he will be overly receptive of metro presoaked food, either. If it was my fish I would simply attempt to get as much into him as possible, via whatever food or method works best. Presoaked pellets, gel food, hex-shield, whatever. You're lucky in so much as your fish is still eating. If things progress, some fish simply stop eating and slowly waste away as they become overcome with the parasites. Others recover but are left with sores in the head region. And of course you can also try treating the water with metro. If he is passing clear waste on a daily basis, while still eating, you are right in that he still has intestinal issues.
 
Unless my old eyes are worse than I thought, his fish appears to have holes located above the sensory pits as well.
 
The ones on the sides are definitely normal pits but the single spot I'm referring too is the one in the center of the head above the eyes.

I am 99% sure that it's not a put as I couldn't find other terrors with the center pit showing like my guy has. Plus with the clear poops being persistent its pretty safe bet it's hith.

Also the fact that it showed up 1 week previous I have a icture that shows a clear forehead. The eye injury he head a week before the spot showed up would be in Kai's case the trigger for the hith I believe.
 
For now I would be more worried about the clear waste, than the pit on his head. The fact that it has become systemic, and travelled to his head region, pretty much confirms Spironucleus vortens.
